A. Larry Wright DBA Wright Roofing & Construction worksite located at 923 W. Bancroft, Toledo, Ohio: On or about December 23, 2014, the employer did not ensure employees were provided with proper eye protection while operating pneumatic nail guns to secure asphalt shingles. Two employees were exposed to potential flying object hazards while operating the pneumatic fasteners.

29 CFR 1926.501(b)(13): Each employee engaged in residential construction activities 6 feet (1.8 m) or more above lower levels was not protected by guardrail systems, safety net system, or personal fall arrest system unless another provision in paragraph (b) of this section provides for an alternative fall protection measure.    A. Larry Wright DBA Wright Roofing & Construction worksite located at 923 W. Bancroft, Toledo, Ohio: On or about December 23, 2014, the employer did not ensure employees were protected from falls while installing weatherproofing on the two story residential structure. Employees were exposed to a fall distance of approximately 20.00 feet from the 6/12 pitch roof.

The program shall enable each employee to recognize the hazards of falling and shall train each employee in the procedures to be followed in order to minimize these hazards.    A. Bancroft, Toledo, Ohio: On or about December 23, 2014, the employer had not trained employees in the appropriate use of fall protection, such as but not limited to, the use of personal fall arrest systems while performing various roofing operations.
