On or about 12/29/14 located at 6120 Formosa LN., Cumming, GA: Employees were observed installing plywood on the roof with a pneumatic nail gun and cutting lumber with a skill saw for the roof sheeting of a new house without safety glasses.

On or about 12/29/14 located at 6120 Formosa LN., Cumming, GA: An employee was installing plywood on the 2nd floor roof ridge top while working at the edge, the employer did not ensure that fall protection was used by the employees while exposed to fall hazards of approximately up to 35 feet.
