29 CFR 1910.305(b)(2)(i): Pull boxes, junction boxes, and fittings were not provided with covers approved for the purpose:  a) Warehouse - On or about 12/29/14 and at times prior thereto the electric box mounted on the wall between the work benches and the electrical panels did not have a cover. The splice was not insulated and copper live parts were exposed.  b) Restroom - On or about 12/29/14 and at times prior thereto the electric box mounted on top of the water heater did not have a cover.
