29 CFR 1910.303(g)(2)(i): Except as elsewhere required or permitted by Subpart S of Part 1910, live parts of electric equipment operating at 50 volts or more were not guarded against accidental contact by use of approved cabinets or other forms of approved enclosures or by any of the means identified in paragraphs (A), (B), (C), and (D) of 29 CFR 1910.303(g)(2)(i): a) 2500-D Warwick Blvd. Newport News, VA 23607 at Newport News Marine Terminal Warehouse B: On or about 12/29/2014 and times prior thereto, employees performed worked, such as but not limited to operating bay door control buttons and sweeping, in the very near close proximity of exposed live electrical wiring of an uncovered electrical fixture, exposing employees to potential electrical shocks and burns. No abatement certification or documentation required.

29 CFR 1910.305(g)(2)(iii): Flexible cords and cables were not connected to devices and fittings so that strain relief was provided that would prevent pull from being directly transmitted to joints or terminal screws: a) 2500-D Warwick Blvd. Newport News, VA 23607 at Newport News Marine Terminal Warehouse A: On or about 12/29/2014 and times prior thereto, employees operated an E-Z-GO Car Division Automatic Charging System to re-energize the battery system on a golf-cart. The flexible cord that entered into the cabinet of the Charging System was missing strain relief protection, exposing employees to potential electrical shocks and burns. Abatement certification required within 10 days after abatement date. The certification shall include a statement that abatement is complete, date and method of abatement, and states employee(s) and their representatives were informed of this abatement.

29 CFR 1917.122(c): Exit(s) were not readily accessible and sufficient in number to provide employees with a convenient means of escape in emergencies. A clear passage to the exit shall be maintained; a) 2500-D Warwick Blvd. Newport News, VA 23607 at Newport News Marine Terminal Warehouse B: On or about 12/29/2014 and times prior thereto, three emergency exits were blocked that would prevent employees from a clear passage to the exit: Area 1: Located on the loading dock side of warehouse B, at or near the center of the structure close to the rail car pit. The area was blocked with pallets of cardboard. Area 2: Located at the dock side of warehouse B, at or near the south-east corner of the building. Area was blocked with piles of lumber including one pallet of wooden blocks. The exit path was cordoned off with railroad tie-style lumber permanently affixed to the floor. This lumber that was bolted to the floor and debris such as PVC pipes created a tripping hazard in the exit path. Area 3: Located at the dock side of warehouse B, at or near the south-west corner of the building, lumber blocked the exit path, exposing employees to tripping injuries such as cuts and contusions. Abatement certification required within 10 days after abatement date. The certification shall include a statement that abatement is complete, date and method of abatement, and states employee(s) and their representatives were informed of this abatement.
