29 CFR 1926.451(g)(1): Each employee on a scaffold more than 10 feet (3.1 m) above a lower level was not protected from falling to that lower level.     A. Michael G. Pass LLC worksite located at 8701 Palermo, Monclova, Ohio: On or about December 30, 2014, the employer did not ensure an employee was protected from a fall hazards while working from a platform secured to a rough terrain lift while performing framing activities. The employee was exposed to a fall distance up to 25.00 feet from the unprotected front side of the platform.

29 CFR 1926.501(b)(13): Each employee engaged in residential construction activities 6 feet (1.8 m) or more above lower levels was not protected by guardrail systems, safety net system, or personal fall arrest system unless another provision in paragraph (b) of this section provides for an alternative fall protection measure.    A. Michael G. Pass LLC worksite located at 8701 Palermo, Monclova, Ohio: On or about December 30, 2014, the employer did not ensure employees were protected from falls while performing framing activities on the two story residential structure. The employee was exposed to a fall distance of approximately 18.00 feet from the southeast corner of the structure while standing on the exterior top plate.
