On or about December 30, 2014, at 1256 Hill Road North, Pickerington Ohio 43147, the employer failed to provide personal protective equipment to ensure an employee working on the roof overhang was protected from falling from the roof, thereby exposing the employee to a 13 foot, 4 inch fall hazard.
