Facility-wide - On or about November 1 through December 18, 2014 the employer required his employees to wear hard hats, face shields, work clothing, dust masks, gloves, and steel toed safety shoes without certifying a hazard assessment had been performed.

29 CFR 1910.134(g)(2)(i): Appropriate surveillance shall be maintained of work area conditions and degree of employee exposure. When there is a change in work area conditions or degree of employee exposure that affected respirator effectiveness, the employer shall reevaluate the continued effectiveness of the respirator. Facility-wide- On or about November 1 through December 18, 2014 neither of the employees working on the lead containing electrical cable wore any respirator routinely but generally used a full face shield which offers no respiratory protection.
