29 CFR 1926.200(g)(2): Traffic control signs or devices used for protection of construction workers did not conform to Part VI of the Manual of Uniform Traffic Control Devices (AMUTCD"), 1988 Edition, Revision 3, September 3, 1993, FHWA-SA-94-027 or Part VI of the Manual on Uniform Traffic Control Devices, Millennium Edition, December 2000, FHWA: a) On or about December 10, 2014, and at times prior, Colorado Barricade Company, as a controlling employer, did not ensure that employees of Traffic Control Staffing were protected from traffic hazards as follows: 1. The employer did not ensure that advance warning signs indicating the presence of flaggers on Mississippi Avenue and S. Willow Street were in place and visible to traffic. 2. Adequate devices and signage were not in place to effectively channel traffic on Mississippi Avenue (east of Valentia St.) from two lanes to one lane. An effective taper had not been implemented and did not match the traffic control plan created by Colorado Barricade Co.
