29 CFR 1910.1200(e)(1)(ii): The written hazard communication program did not include the methods the employer would use to inform employees of the hazards of non-routine tasks, and the hazards associated with chemicals contained in unlabeled pipes in their work areas:         1.) On or about December 30, 2014 The employer did not ensure that directions for conducting non-routine tasks during an employee's work schedule were included in the hazard communication program.

29 CFR 1910.1200(h)(1): Employers did not provide employees with effective information and training on hazardous chemicals in their work area at the time of their initial assignment, and whenever a new chemical hazard the employees have not previously been trained about is introduced into their work area. Information and training may be designed to cover categories of hazards (e.g., flammability, carcinogenicity) or specific chemicals. Chemical-specific information must always be available through labels and safety data sheets.       1.) On or about December 30, 2014, the employer failed to train employees on the elements of a written hazard communication program including the hazards of the chemicals in their work area.  Employees were exposed to hazards from carbon monoxide gas from the exhaust of a propane powered floor buffer used in the facility.
