29 CFR 1926.501(b)(4)(i):  Each employee on walking/working surfaces was not protected from falling through holes (including skylights) more than 6 feet (1.8 m) above lower levels, by personal fall arrest systems, covers, or guardrail systems erected around each hole:    a)  Jobsite, First Floor, 385 South Bouquet Street, Pittsburgh, PA 15213:  Observed on or about December 30, 2014, each employee on a walking/working surface was not protected from falling through an open floor hole.  A floor opening on the first floor measured approximately 3 feet wide and 12 feet long.  Employees engaged in framing activities on the first floor of the duplex were exposed to a fall hazard of approximately 10 feet to the unfinished (gravel) basement floor below.     b)  Jobsite, Second Floor, 385 South Bouquet Street, Pittsburgh, PA 15213:  Observed on or about December 30, 2014, each employee on a walking/working surface was not protected from falling through an open floor hole.  A floor opening on the second floor measured approximately 3 feet wide and 7 feet long.  Employees engaged in framing activities on the second floor of the duplex were exposed to a fall hazard of approximately 20 feet to the unfinished (gravel) basement floor below.

29 CFR 1926.501(b)(14):  Each employee working on, at, above, or near wall openings (including those with chutes attached), where the outside bottom edge of the wall opening was 6 feet (1.8 m) or more above lower levels and the inside bottom edge of the wall opening was less than 39 inches (1.0 m) above the walking/working surface, was not protected from falling by the use of a guardrail system, a safety net system, or a personal fall arrest system:      a)  Jobsite, Second floor window facing Bouquet Street, 385 South Bouquet Street, Pittsburgh, PA 15213:  Observed on or about December 30, 2014, each employee  walking/working on, at or above wall openings where the outside bottom edge of the wall opening was greater than 6 feet or more above lower levels were not protected from falling by the use of a guardrail system, a safety net system, or a personal fall arrest system.  The inside bottom edge of the window measured approximately 36 inches above the floor.  Employees engaged in framing activities on the second floor of the duplex were exposed to a fall hazard of approximately 13 feet to the ground below.    b)  Jobsite, Second floor window on opposite side of Bouquet Street, 385 South Bouquet Street, Pittsburgh, PA 15213:  Observed on or about December 30, 2014, each employee walking/working on, at or above wall openings where the outside bottom edge of the wall opening was greater than 6 feet or more above lower levels were not protected from falling by the use of a guardrail system, a safety net system, or a personal fall arrest system.  The inside bottom edge of the window measured approximately 36 inches above the floor.  Employees engaged in framing activities on the second floor of the duplex were exposed to a fall hazard of approximately 25 feet to the ground below.

29 CFR 1926.503(b)(1):  The employer did not verify compliance with paragraph (a) of this section by preparing a written (training) certification record including the name or other identity of the employee trained, the date(s) of the training, and the signature of the person who conducted the training or the signature of the employer:  a)  Jobsite, 385 South Bouquet Street, Pittsburgh, PA 15213:  Observed on or about December 30, 2014, the employer did not maintain a written certification record of fall protection training for each employee working at the jobsite.  Two employees engaged in framing activities on the second floor of a two story duplex were exposed to fall hazards.  The employer did not have a written certification of fall protection training for either employee.
