OSH ACT of 1970 Section (5)(a)(1): The employer did not furnish employment and a place of employment which was free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to struck-by and crushed-by hazards: a) Arrowhead Landfill; On or about December 31, 2014 and at times prior, the employer did not ensure that employees did not exit the cab of the bulldozer without first completing steps as required by the equipment manufacturer such as but no limited to engaging the parking brake, lowering the blade and or attachments to the ground, locking the hydraulic system and turning off the engine.
