29 CFR 1926.501(b)(4): Each employee on walking/working surfaces was not protected from falling through holes, more than six feet above lower levels, by personal fall arrest systems, covers, or guardrail systems erected around such holes:    a) 530 N 36th St, Philadelphia, PA - The employer did not ensure that each employee was protected from falling through the hole in the floor on the walking/working surface by installing some type of fall protection system. This hazard exposed employees to a fall hazard. This condition was observed on or about December 10, 2014.

29 CFR 1926.501(b)(13): Each employee engaged in residential construction activities 6 feet or more above lower levels was not protected by a guardrail system, safety net system, or personal fall arrest system, or an alternative fall protection measure:    a) 530 N 36th St, Philadelphia, PA - The employer did not ensure that each employee on the residential construction site was protected from falling from heights ranging from approximately 20 to 30 feet by the use of or installing some type of fall protection system. This condition was observed on or about December 10, 2014.

29 CFR 1926.501(b)(14): The employer did not ensure that each employee working on, at, above, or near wall openings (including those with chutes attached) where the outside bottom edge of the wall opening was 6 feet or more above lower levels and the inside bottom edge of the wall opening was less than 39 inches above the walking/working surface, were not protected from falling by the use of a guardrail system, a safety net system, or a personal fall arrest system.    a) 530 N 36th St, Philadelphia, PA - The employer did not ensure that the employees were protected from falling through wall openings ranging from approximately 10 feet to 20 feet above the ground thus exposing the employees to a fall hazard. A fall protection system was not used to protect the employees from falling to the next level. This condition was observed on or about December 10, 2014.

29 CFR 1926.503(a)(1): The employer did not provide a training program for each employee exposed to fall hazards to enable them to recognize the hazards of falling and the procedures to follow in order to minimize them.  530 N 36th St, Philadelphia, PA - Employees were not instructed in the recognition and avoidance of fall hazards and the regulations of their work environment to control or eliminate hazards. The employees were exposed to fall hazards greater than 6 feet while performing framing operations on or about December 10, 2014.
