29 CFR 1926.100(a): Employees working in areas where there was a possible danger of head injury from impact, or falling or flying objects, or from electrical shock and burns, were not protected by protective helmets:   (a) Jobsite:  On or about January 2, 2015, employees were not wearing head protection (hard hats) while they were working from the ground and other employees were working from the roof.  In addition, one employee was not wearing a hard hat while she was working under a partially demolished and rotten back porch roof.

29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b):      (a)  Jobsite, Roof:  On or about January 2, 2015 an employee was working on the roof and he was not protected from falling to the ground.  The fall hazard to the employee was about 27 feet.  The pitch of the roof was about 2 in 12.
