Section 5(a)(1): The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to potential crushing hazards from falling loads and/or lift parts:     (a) On or about December 18, 2014, and at times prior to that date, for employees working around and under lift #1, Challenger, two-post lift, Model #VS10, Serial #080200000040515, 10,000 pounds capacity and exposed to struck-by hazards where a lift anchor bolt was not properly secured to the concrete floor and the west side safety locks would not engage at The Hersrud's Company  located at 2651 Whitewood Road in Sturgis, South Dakota.     (b)   On or about December 18, 2014, and at times prior to that date, for employees working around and under Lift #2, Weaver in-ground, Model #EC401, Serial #4AH119, and exposed to struck-by hazards where there was no mechanism in place to lock the lift and extension arms were bent while vehicles were elevated at The Hersrud's Company located at 2651 Whitewood Road in Sturgis, South Dakota.     (c)   On or about December 18, 2014, and at times prior to that date, for employees working around and under Lift #4, Weaver in-ground, Model #EC401, Serial #4AH119, and exposed to struck-by hazards where there was no mechanism in place to lock the lift while vehicles were elevated and a wood block was used to allow vehicles to be lowered while the controls were unattended at The Hersrud's Company located at 2651 Whitewood Road in Sturgis, South Dakota.     (d)   On or about December 18, 2014, and at times prior to that date, for employees working around and under Lift #5, Challenger two-post, #CL9, Serial #902167, and exposed to struck-by hazards where the rear lateral arm restraint teeth were not making contact with the hoist frame and the east side safety locks would not fully engage at The Hersrud's Company located at 2651 Whitewood Road in Sturgis, South Dakota.     (e)   On or about December 18, 2014, and at times prior to that date, for employees working around and under Lift #6, Weaver in-ground, Model #EC401, and exposed to struck-by hazards where there was no mechanism in place to lock the lift while vehicles were elevated at The Hersrud's Company located at 2651 Whitewood Road in Sturgis, South Dakota.     (f)   On or about December 18, 2014, and at times prior to that date, for employees working around and under Lift #7, Challenger two-post, Model #E12, Serial #111200000074062, and exposed to struck-by hazards where vehicles' clearance has not been met with a gas-forced air heater while vehicles are raised on the lift and post jacks were used to support vehicles while on the lift at The Hersrud's Company located at 2651 Whitewood Road in Sturgis, South Dakota.     (g)   On or about December 18, 2014, and at times prior to that date, for employees working around and under Lift #8, Weaver in-ground, Model #EC401, and exposed to struck-by hazards where there was no mechanism in place to lock the lift while vehicles were elevated and a wood block was used to allow vehicles to be lowered while the controls were unattended at The Hersrud's Company located at 2651 Whitewood Road in Sturgis, South Dakota.     Abatement Note:  Among other methods, one feasible and acceptable method of abatement would be to follow the requirements of ANSI/ALI ALOIM and/or ANSI B153.1 which includes, among other requirements, the following:        1.   Institute a preventive maintenance program according to the manufacturer's specifications.        2.   Provide training to employees on proper use and care of the lifts.        3.   Ensure all lifts are rated and marked with their load capacity.        4.   Schedule and complete all periodic and annual inspections.
