29 CFR 1926.100(a): Employees working in areas where there was a possible danger of head injury from impact, or falling or flying objects, or from electrical shock and burns, were not protected by protective helmets.  On or about 05 January 2015, and at times prior thereto, an employee without head protection was working below other employees performing framing activities, exposing the employee to the hazard of being struck by falling or flying objects, materials and tools.

29 CFR 1926.102(a)(1): Eye and face protective equipment were not used when machines or operations presented potential eye or face injury from physical, chemical, or radiation agents.  On or about 05 January 2015, two employees were using pneumatic nail guns to perform framing activities without the use of eye protection, exposing the employee to the hazard of being struck by flying objects.

29 CFR 1926.451(a)(6): Scaffolds were not constructed in accordance with the design by a qualified person.  a) On or about 05 January 2015 an employee was performing framing activities from a temporary elevated work platform on the forks of a rough terrain forklift up to approximately 16 feet above the ground without the scaffold being erected by a qualified person, exposing the employee to a fall hazard.  b) On or about 05 January 2015 an employee was performing framing activities from makeshift scaffolding erected around the exterior of the building under construction up to approximately 16 feet above the ground without the scaffold being erected by a qualified person, exposing the employee to a fall hazard.

29 CFR 1926.1053(b)(4): Ladders were used for purposes other than the purposes for which they were designed.  On or about 05 January 2015, a worker was using an 8-foot, portable, free-standing ladder that was folded and leaned against the building to pass materials up to an employee above, exposing the worker to a fall hazard of approximately 6 feet.
