29 CFR 1926.21(b)(2): The employer shall instruct each employee in the recognition and avoidance of unsafe conditions and the regulations applicable to his work environment to control or eliminate any hazards or other exposure to illness or injury: On or about January 5, 2015, the employer did not provide training for two employees performing activities including, but not limited to, working with an unguarded 14-inch Archer Speed abrasive cut off wheel (AMC 114B/A36, 14x7/64 x 1) attached to a 7-inch Bosch angle grinder; employees were exposed to struck-by hazards.

29 CFR 1926.300(b)(1): Power operated tool(s), designed to accommodate guards, were not equipped with such guards when in use: On or about January 5, 2015, on the backside of a newly constructed apartment complex, an employee was observed working with an unguarded 14-inch Archer Speed abrasive cut off wheel (AMC 114B/A36, 14x7/64 x 1) attached to a 7-inch Bosch angle grinder. The employee was using the angle grinder to cut metal and was exposed to struck-by hazards.
