29 CFR 1926.100(a): Employees working in areas where there was a possible danger of head injury from impact, or falling or flying objects, or from electrical shock and burns, were not protected by protective helmets:   Employees were performing a roofing job at 1011 W. 10th Street in Topeka, Kansas.  An employee with no hard hat was observed working on the ground exposed to head injury from falling objects from the roof.

29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b):     Employees were performing a roofing job at 1011 W. 10th Street in Topeka, Kansas.  Employees were exposed to fall hazards of approximately 18 feet from the flat roof portions to the ground below in that no fall protection was used.   Midwest Coating, Inc. was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1926.501(b)(10), which was contained in OSHA inspection number 314028572 (OIS #173712), citation number 1, item number 1, and was affirmed as a final order on 11/08/2010, with respect to a workplace located at 5257 SW 33rd Street, Topeka, Kansas.

29 CFR 1926.1053(b)(1): Where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed:   Employees were performing a roofing job located at 1011 W. 10th Street in Topeka, Kansas.  A portable extension ladder used to access the roof had side rails which extended approximately 2 feet 6 inches above the landing surface of the roof.
