29 CFR 1910.1200(e)(1): Employers did not develop, implement, and maintain at each workplace, a written hazard communication program which at least describes how the criteria specified in paragraphs (f), (g), and (h) of this section for labels and other forms of warning, material safety data sheets, and employee information and training were met (Referenced in 29 CFR 1926.59): a) On January 28, 2015, a written hazard communication program was not developed for employees that worked with chemicals to include, but not limited to enamel paints, including Krylon spray cans, acid cleaner, pipe cleaner, pipe glue, roof cement, and solvents.

29 CFR 1910.303(g)(1)(i)(B): For electric equipment operating at 600 volts, nominal, or less to ground, and likely to require examination, adjustment, servicing, or maintenance while energized, the width of the working space in front of electrical equipment was less than the width of the equipment or 762 mm (30 inches), whichever was greater : a) On January 28, 2015, inside the maintenance shop, the electrical controller labeled "Green Pump", operating at 240 volts was blocked by debris and equipment.
