29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b): a) On December 3, 2014 and at times prior thereto; upper platform; employees framing a new extension on a residential home were observed walking the outer edge of the platform without fall protection in place to prevent them from falling 25 feet to ground level.

29 CFR 1926.1053(b)(1): Where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed. a) On December 3, 2014 and at times prior thereto; second floor north-side; the six foot step ladders used to access the platform area were not tall enough to extend three feet above the working platform exposing them to a 9 foot fall hazard while using the ladder to access the platform.

29 CFR 1926.1053(b)(13): The top or top step of a stepladder was used as a step: a) On December 3, 2014 and at times prior thereto; second floor north-side; the six foot step ladders used to access the platform area were not tall enough to extend three feet which resulted in the employees using the top step while accessing the platform area or while stretching from the platform area to access the top of the ladder; exposing employees to a fall hazard of 9 feet to the lower platform floor.
