29 CFR 1910.212(a)(1): One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks:  On or about January 06, 2015 and at times prior thereto, the employer did not ensure that machine guarding was provided in four instances within the facility:  a. In the radiator cutting area the Gensco Hydraulic Shears DTX 600 adjustable guard was not lowered into place, exposing employee to the hazard of being caught in the shears' blade.   b. In the bag house area the Gensco Hydraulic Shears DTX 400 adjustable guard was not lowered into place, exposing employee to the hazard of being caught in the shears' blade.  c. In the Mayco Lead Processing area the #8 Catalytic Shears did not have a guard installed, exposing employee to the hazard of being caught in the shears' blade.  d. In the radiator cutting area the DeWalt D28715 Chop Saw was missing the abrasive cutting wheel guard, exposing employees to the hazard of being struck by the saw blade.
