29 CFR 1910.303(b)(2): Listed or labeled electrical equipment was not used or installed in accordance with instructions included in the listing or labeling:    (a) Saw Room -On or about 1/14/15, a metal junction box with pre-punched holes was not mounted in accordance with the listing for this type of equipment.  This junction box exposed employees to an electrical shock hazard as it was observed dangling down along a cinder block wall.         Pursuant to 29 CFR 1903.19, the employer must submit abatement certification and documentation, required within 10 days after abatement date. The certification shall include a statement that abatement is complete, the date and method of abatement, and state that employees and their representatives were informed of this abatement. Abatement documentation shall include documents demonstrating that abatement is complete, such as evidence of the purchase or repair of equipment, photographic or video evidence of abatement or other written records.

The employees were exposed to an amputation hazards while using a table saw, which was set up in an angle to do mitered cuts of composite plastics, and cardboard pipes, without guarding the point of operation.
