29 CFR 1903.19(d)(1): The employer did not submit to the Agency documents demonstrating that abatement is complete for each willful or repeat violation and for any serious violation for which the Agency indicates in the citation that such abatement documentation is required. (a) On or about January 7, 2015, the employer failed to keep the front passageway orderly by having a 1/2 inch steel plate protrude approximately one foot into a working passageway, exposing the employees to a tripping hazard; (b) On or about January 7, 2015, the employer failed to clearly mark the emergency exit with a visible sign reading EXIT; (c) On or about January 7, 2015, the employer failed to assure that the two portable fire extinguishers in the facility had annual maintenance checks; (d) On or about January 7, 2015, the employer failed to properly affix the guard to the meat slicer. The bolts holding the guard in place were damaged and missing; (e) On or about January 7, 2015, the employer failed to guard the 120 volt live wires of a Hill Cooler located in the front middle of the sales floor against accidental contact.
