29 CFR 1926.405(g)(1)(ii): Flexible cord was not equipped with an attachment plug and energized from a receptacle outlet when used as permitted in 29 CFR 1926.405(g)(1)(i) [C], [F], or [H]:  a. The employer did not ensure that a flexible cord was equipped with an attachment plug and energized from a receptacle outlet when used as permitted in 29 CFR 1926.405(g)(1)(i) [C], [F], or [H].  This violation occurred on or about January 7, 2015, when the employer allowed employees to use temporary lighting that was plugged into a receptacle without an attachment plug, but rather with the individual conductors inserted into the receptacle.
