29 CFR 1926.451(b)(2)(ii): When walkways between scaffold sections are less than 18 inches wide, employees were not protected from falls by guardrail systems or personal fall arrest systems:   a) Scaffold system at Sleep Inn and Suites: Front end of the building: On or about January 7, 2015 one employee was observed working/walking on scaffold platform which was 12 inches wide. The employee was not protected from falls by guardrail systems or personal fall arrest systems.

29 CFR 1926.451(b)(5)(i): The end of a platform 10 feet or less in length extended over its support more than 12 inches and the platform was not designed and installed so that the cantilevered portion of the platform is able to support employees and/or materials without tipping, or has guardrails which block employee access to the cantilevered end.   a) Fabricated Scaffold at Sleep Inn and Suites: Front end of the building: On or about January 7, 2015 one employee was observed working/walking on scaffold platform  the end of the platform extended over its support more than 12 inches and the platform was not designed and installed so that the cantilevered portion of the platform was able to support employee and /or materials without tipping or had guardrails which block employee access to the cantilevered end.

29 CFR 1926.451(e)(1): When scaffold platforms were more than 2 feet (0.6 m) above or below a point of access, portable ladders, hook-on ladders, attachable ladders, stair towers (scaffold stairways/towers), stairway-type ladders (such as ladder stands), ramps, walkways, integral prefabricated scaffold access, or direct access from other scaffold, sturcure, personnel hoist, or similar surface was not used.    a) Fabricated scaffold at Sleep Inn and Suites: Front end of the building: On or about January 7, 2015 one employee was observed working/walking on scaffold platform which was more than 2 feet above a point of access. The scaffold did not have a ladder or other safe means of access to the platform.

29 CFR 1926.451(f)(7): Scaffolds were not erected, moved, dismantled, or altered, by trained and experienced employees under the supervision and direction of a competent person qualified in scaffold erection, moving, dismantling or alteration:; Such activities were not performed only by experienced and trained employees selected for such work by the competent person.   a) Fabricated Scaffold: On or about January 7, 2015 and at times prior thereto, the employer did not ensure a competent person was onsite to supervise the erection of the scaffolding system which exposed employees to scaffolding related hazards such as but not limited to falls up to 8-feet.
