29 CFR 1910.119(e)(3)(vii): The employer did not ensure that the process hazard analysis (PHA) had a complete qualitative evaluation of a range of the possible safety and health effects of failures of controls: 35955 Navaho Rd., Ravenna, NE 68869 (Jobsite): The employer is failing to ensure that the process hazard analysis (PHA) had a complete qualitative evaluation of a range of the possible safety and health effects of failures of controls. The process hazard analysis (PHA) was completed on September 2, 2014. However, the PHA did not fully address qualitative evaluation of a range of the possible safety and health effects of failures of controls including, but not limited to; having an incomplete evaluation on the PHA Unit: Rectifier. There was an evaluation of the loss of containment; however, the consequences in regard to the possible injury did not detail what the actual injuries could be. Abatement certification is required for this violation. The abatement certification sheet is enclosed with the citations.

29 CFR 1910.151(c): Where employees were exposed to injurious corrosive materials, suitable facilities for quick drenching or flushing of the eyes and body were not provided within the work area for immediate emergency use: 35955 Navaho Rd., Ravenna, NE 68869 (Corn Oil Building): The employer is failing to ensure that where employees were exposed to injurious corrosive materials, suitable facilities for quick drenching or flushing of the eyes and body was provided within the work area for immediate emergency use. An employee had chemical burns when exposed to corn/sorghum distillers solubles. The employee had to use an eyewash station that was located in the lab, which was approximately 80 feet away. Abatement certification is required for this violation. The abatement certification sheet is enclosed with this citation.
