29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b):     Employees performing roofing work on a roof 6 feet or more above ground are not protected from fall hazards by a fall protection system.     a. The employer fails to assure effective protection against falls as required by the standard, as demonstrated on 01/07/2015 when an employee was working on a 6 in 12 pitched residential roof at Lot #83 on Umbria Lane, Saint Cloud, FL 34771 without fall protection and was exposed to falls of 17 feet. The employee installed three roof anchors while not being tied off.     b. The employer fails to assure effective protection against falls as required by the standard, as demonstrated on 01/07/2015 when an employee was working on a 6 in 12 pitched residential roof at Lot #83 on Umbria Lane, Saint Cloud, FL 34771 without fall protection and was exposed to falls of 10 feet. The employee was passing rolls of felt roofing material on to the top roof from the lower gable.    Collis Roofing, Inc. was previously cited for a violation of this Occupational Safety and Health Standard or its equivalent standard 29 CFR 1926.501(b)(13) which was contained in OSHA Inspection Number 739143, Citation 01 Item 001 and was affirmed as a final order on May 23, 2013, with respect to workplace located at 256 Chandler, Oviedo, FL 32765.    Collis Roofing, Inc. was previously cited for a violation of this Occupational Safety and Health Standard or its equivalent standard 29 CFR 1926.501(b)(13) which was contained in OSHA Inspection Number 472520, Citation 01 Item 001 and was affirmed as a final order on August 29, 2012, with respect to workplace located at 2120 White Jasmine Ct. Apopka, FL 32703.    Collis Roofing, Inc. was previously cited for a violation of this Occupational Safety and Health Standard or its equivalent standard 29 CFR 1926.501(b)(11) which was contained in OSHA Inspection Number 315570697, Citation 01 Item 001 and was affirmed as a final order on May 17, 2011, with respect to workplace located at 136 N. 4th St. Lake Mary, Florida 32746.    Collis Roofing, Inc. was previously cited for a violation of this Occupational Safety and Health Standard or its equivalent standard 29 CFR 1926.501(b)(13) which was contained in OSHA Inspection Number 315498089, Citation 01 Item 001 and was affirmed as a final order on May 18, 2011, with respect to workplace located at 4257 Isle Vista Ave., Belle Isle, FL 32812.    Collis Roofing, Inc. was previously cited for a violation of this Occupational Safety and Health Standard or its equivalent standard 29 CFR 1926.501(b)(13) which was contained in OSHA Inspection Number 315479931, Citation 01 Item 001 and was affirmed as a final order on December 28, 2011, with respect to workplace located at 3820 Catania Place, Jacksonville, FL 32224.
