29 CFR 1926.1053(b)(1): When portable ladders were used for access to an upper landing surface, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface to which the ladder is used to gain access. On or about 1/07/2015, at the above addressed work site, a portable extension ladder did not extend at least 3-feet above the roof landing exposing employees to a fall hazard of approximately 10-feet.

29 CFR 1910.1200(e)(1): the Employer did not develop, implement, and maintain at each workplace, a written hazard communication program which at least describes how the criteria specified in paragraphs (f), (g), and (h) of this section for labels and other forms of warning, material safety data sheets, and employee information and training will be met. On or about 01/07/2015, at the above addressed worksite, the employer did not have a hazard communication program to inform employees of the hazards when using chemicals such as paint and Xylene.
