The separator was contaminated with residues from circulation through the hole including, but not limited to, diesel-based mud, crude oil, and hydrocarbons, exposing employees to potential oxygen deficient atmospheres and/or inhalation of toxic gases or vapors in the event of entering the tank.

The separator was contaminated with chemical residues from circulation through the hole including, but not limited to, diesel-based mud, crude oil, and hydrocarbons, exposing employees to potential oxygen deficient atmospheres and / or inhalation of toxic gases or vapors in the event of entering the tank.
