29 CFR 1910.119(d)(3)(ii): The employer shall document that equipment complies with recognized and generally accepted good engineering practices. On February 4, 2015, the employer did not document that its equipment complies with recognized and generally accepted good engineering practice according to the ASHRAE-15 and IIAR-2 ANSI Standards or equivalent. Examples include, but not limited to: a. Engine Room No. 1 - The "West" double doors and the "North" door were not tight fitting, permitting passage of escaping ammonia to other work areas. b. Engine Room No. 2 - The "East" door was not tight fitting, permitting passage of escaping ammonia to other work areas. c. Engine Room No. 1, "West" and "North" doors - emergency shutdown procedures including precautions to be observed in case of breakdown, or leak, were not posted outside of each engine room, immediately adjacent to each door. d. Engine Room No. 2, "North" and "East" doors - emergency shutdown procedures including precautions to be observed in case of breakdown, or leak, were not posted outside of each engine room, immediately adjacent to each door. f. Engine Room No. 1, outside the "North" door there were no visual and audible ammonia alarms. Also the meaning of each alarm was not marked by signs near the annunciators. g. East side of building - On the "Northeast" door, inside and outside Engine room No. 1 there were no visual and audible ammonia alarms. Also the meaning of each alarm was not marked by signs near the annunciators. h. West side of building - On the "West", "East" and "North" doors inside and outside Engine room No. 2 there were no visual audible and ammonia alarms. Also the meaning of each alarm was not marked by signs near the annunciators. i. East side of building - On the "North" door of Engine Room No. 1 there were no legible and permanent sign, securely attached and easily accessible, reading "Machinery Room - Authorized Personnel Only", and further communicating that entry is forbidden except by those personnel trained in emergency procedures when the ammonia alarm is activated. j. West side of building - On the "North" and "East" doors of Engine Room No. 2 there were no legible and permanent sign, securely attached and easily accessible, reading "Machinery Room - Authorized Personnel Only", and further communicating that entry is forbidden except by those personnel trained in emergency procedures when the ammonia alarm is activated. k. Ammonia Refrigeration Room No. 2 did not have a manual reset for alarms located inside the room. Aryzta LLC was previously cited for a violation of this occupational safety and health standard or its equivalent standard (1910.119(d)(3)(ii) which was contained in OSHA inspection number 870303, citation number 1, item number 1, issued on July 29, 2013 and became final order on August 16, 2013, with respect to a workplace located at 111 Northwest Avenue, Northlake, Illinois 60164.
