29 CFR 1910.252(a)(1)(ii): When the object to be welded or cut could not be moved and all the fire hazards could not be removed, guards were not used to confine the heat, sparks, and slag to protect the immovable fire hazards: Technisand Plant # 2. Worksite located at 900 S. Hickory. Roff, OK 74865. On or about January 6, 2015, employees were welding on the dust collection system which contained a fire hazard without the use of a guard to confine the heat, sparks, and or slag. The employees were exposed to explosion and fire hazards.

29 CFR 1910.22(a)(1): All places of employment, passageways, storerooms or service rooms were not kept clean and orderly or in a sanitary condition. Technisand Plant # 2. Worksite located at 900 S. Hickory. Roff, OK 74865. On or about January 7, 2015, and at times prior thereto, the floors in Plant # 2 were not kept in a clean and orderly condition. Sand and other debris was on the floor exposing employees to slip and trip hazards.
