29 CFR 1910.269(p)(3): Mechanical equipment used to lift or move lines or other material was not used within its maximum load rating and other design limitations for the conditions under which the mechanical equipment was being used: "On or about January 6, 2015, the Caterpillar 950K wheeled loader with forklift tine attachment was used to hold and guide a wooden power pole as it was being cut and felled under conditions exposing an employee to struck-by hazard."
