(b)    Fabrication Shop:  A Dake Johnson band saw model number JH10 (Serial number unknown) that was last used to cut one inch pipe did not have a guard installed over the fourteen inch section of the saw blade that was not engaged in the cutting action as observed on January 8, 2015.  ABATEMENT CERTIFICATION REQUIRED
