29 CFR 1926.452(w)(2): Scaffold casters and wheels were not locked with positive wheel and/or wheel and swivel locks, or equivalent means, to prevent movement of the scaffold while the scaffold was used in a stationary manner:   The employer does not ensure that scaffold casters and wheels are locked.  This violation occurred on or about January 7, 2015, on the fourth level of the main office building where an employee was exposed to a fall hazard when using a mobile scaffold to install insulation without all the wheels locked.    Pursuant to 29 C.F. R. 1903.19, within ten (10) calendar days of the abatement date, the employer must submit documentation showing that it is in compliance with the standard, including describing the steps that it is taking to ensure that scaffold casters and wheels are locked.
