29 CFR 1926.651(c)(2): A stairway, ladder, ramp or other safe means of egress was not located in trench excavations that were 4 feet (1.22m) or more in depth so as to require no more than 25 feet (7.62m) of lateral travel for employees: The employer does not ensure that a stairway, ladder, ramp or other safe means of egress is located in trench excavations that are 4 feet or more in depth. This violation occurred on or about January 7, 2015, on the north side of the jobsite where an employee was exposed to a fall hazard when installing rebar in an excavation greater than 4 feet without using a stairway, ladder or ramp to egress from the excavation. Pursuant to 29 C.F. R. 1903.19, within ten (10) calendar days of the abatement date, the employer must submit documentation showing that it is in compliance with the standard, including describing the steps that it is taking to ensure that a stairway, ladder, ramp or other safe means of egress is located in trench excavations that were 4 feet or more in depth.
