29 CFR 1910.22(a)(1): All places of employment, passageways, storerooms or service rooms were not kept clean and orderly or in a sanitary condition: (a) On or about 01/08/2015 - Main shop, combustible dust generated from the wood working, sawing, cutting, planning, and sawmill operation had accumulated on the walls, pipes, and beams exposing employees to a fire hazard. The dust on the pipes measured between 1/8 inch and 3 inches in depth. The dust attached to the walls measured 1/4 inch in depth. The dust under the equipment measured between 2 and 3 inches in depth.

29 CFR 1910.147(c)(1): The employer did not establish a program consisting of an energy control procedure, employee training and periodic inspections to ensure that before any employee performed any servicing or maintenance on a machine or equipment where the unexpected energizing, startup or release of stored energy could occur and cause injury, the machine or equipment shall be isolated from the energy source and rendered inoperative: (a) On or about 01/08/2015 - Facility, the employer did not develop an energy control program that included lockout/tagout procedures, employee training, and periodic inspections for employees performing servicing and maintenance on equipment such as, but not limited to, table saws, band saws, rip saws, and the CNC machine.

29 CFR 1910.307(c): Equipment, wiring methods, and installations of equipment in hazardous (classified) locations were not intrinsically safe, approved for the hazardous (classified) location, or safe for the hazardous (classified) location: (a) On or about 01/08/2015 - Main shop, the 240-volt panel had opening(s) and missing knockout(s) preventing it from being dust-tight. Dust was allowed to accumulate inside the panel. (b) On or about 01/08/2015 - Main shop, the 240 volt outlet was missing the cover preventing it from being dust tight. (c) On or about 01/08/2015 - Main shop, the 240 volt wiring providing power to the Thermwood CNC was not sealed and did not provide a dust tight seal. Dust was allowed to accumulate inside the control panel. (d) On or about 01/08/2015 - Main shop, the 240 volt wiring providing power to the CNC was not sealed and did not provide a dust tight seal. Dust was allowed to accumulate inside the panel.
