29 CFR 1910.212(a)(1): Machine guarding was not provided to protect operator(s) and other employees from hazards created by rotating parts, and/or flying chips:  Outside Mold Making area - Machine guarding was not provided for the Rockwell Drill Press to protect employees from the hazards of flying chips and the rotating spindle, exposing employees to a struck by and caught in hazard.  29 CFR 1903.19(c)(1) requires certification that the abatement of the above violation is complete.

29 CFR 1910.1200(h)(3)(iv): Employees had not been provided with GHS 2012 training including an explanation of the labels received on shipped containers and the workplace labeling system to be used by their employer; the safety data sheet, including the order of information, and how employees can obtain and use the appropriate hazard information:  The employer has not updated the hazard communication program and trained employees on the portion of the hazard communication program that explains the labels on shipping containers and the safety data sheets for items such as 74-Series part A Polyurethane Prepolymer, 74-Series Part B Polyol, oils and fuels, including the order of information and how the employee could obtain and use the hazard information. Employees were not trained regarding the new label elements and safety data sheets format by December 1, 2013.  29 CFR 1903.19(c)(1) requires certification that the abatement of the above violation is complete.

29 CFR 1910.134(c)(2)(i): Respirator users were not provided with the information contained in Appendix D to 29 CFR 1910.134 when the employer determined that any voluntary respirator use was permissible:  Green-ware Room and Rubber Room - Employees used N95 dust masks, but they were not provided with the information in Appendix D.  29 CFR 1903.19(c)(1) requires certification that the abatement of the above violation is complete.
