29 CFR 1904.39(a): The employer did not report the in-patient hospitalization, amputation, or loss of an eye to OSHA within twenty-four (24) hours after the in-patient hospitalization of one or more employees or an employee's amputation or an employee's loss of an eye, as a result of a work-related incident: A work-related incident that resulted in a multi-day hospitalization of the injured employee beginning on 1/4/15, was not reported to OSHA until 1/7/15, which was beyond the 24-hour requirement for notification.
