29 CFR 1910.132(d)(1): The employer did not assess the workplace to determine if hazards are present, or are likely to be present, which necessitate the use of personal protective equipment (PPE):  Food Production Room:  On or about January 6, 2015, and at times prior thereto, employees were exposed to the hazards of a chemical splash to the eyes and skin from the absence of an assessment to determine the need for personal protective equipment such as, but not limited to, goggles, chemical resistant rubber or neoprene gloves, and face shields, while carrying tubs of Disan-1 Disinfectant Sanitizer, a corrosive.

29 CFR 1910.212(a)(1):  One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks:    a)  Masa Room:  Hobart Mixer-Grinder Machine, Model 4356.  On or about January 6, 2015, and at times prior thereto, employees were exposed to caught-in and amputation hazards from the absence of a guard for rotating parts, while operating the mixer-grinder.

29 CFR 1910.303(b)(7)(iv):  A damaged electrical cord was in disrepair, and could adversely affect safe operation or mechanical strength of the equipment, such as parts that are broken, bent, cut, or deteriorated by corrosion, chemical action, or overheating:   Production Room, near south wall, Baldor Motor for Chorizo Machine, Cat No. LDP3443.  On or about January 6, 2015, employees were exposed to the hazard of electrical shock, from a damaged electrical cord jacket on the motor, while processing chorizo.
