29 CFR 1904.39(a)(2): The employer did not report the in-patient hospitalization, amputation, or loss of an eye as a result of a work-related incident to OSHA within twenty-four (24) hours: On or about January 4, 2015, at the job site located at or near 1000 McHenry Street, El Dorado the employer failed to report a work-related in-patient hospitalization event to OSHA within 24 hours of an employee's hospital admission for care or treatment. The in-patient hospitalization was reported to OSHA via the OSHA Hotline at 1-800-321-OSHA (6742) by the employer on January 6, 2015, 2 days after the incident.
