29 CFR 1926.501(b)(1): Each employee on a walking/working surface with an unprotected side or edge which was 6 feet (1.8 m) or more above a lower level was not protected from falling by the use of guardrail systems, safety net systems, or personal fall arrest systems:    a. On or about January 7, 2015, an employee was working from the edge of the roof and did not have any type of fall protection, exposing himself to a fall of 120 feet.

29 CFR 1926.1051(a): Stairways or ladders were not provided at all personnel points of access where there was a break in elevation of 19 inches (48 cm) or more, and no ramp, runway, sloped embankment, or personnel hoist was provided:    a.  On or about January 7, 2015, on the roof, employees climbed to the roof using sprinkler system piping and were not provided safe access exposing them to slip, trip, and fall hazards.

29 CFR 1926.1052(c)(1): Stairways having four or more risers or rising more than 30 inches (76 cm), whichever is less, were not equipped with at least one handrail and one stairrail system along each unprotected side or edge:    a.  On or about January 7, 2015, employees accessing the roof used a set of stairs fourteen risers high without a stairrail system along each unprotected side, exposing themselves to a fall hazard.
