29 CFR 1910.134(c)(1): A written respiratory protection program that included the provisions in 29 CFR 1910.134(c)(1)(i) - (ix) with worksite specific procedures was not established and implemented for required respirator use:         a)  Mixing Area, an employee in the mixing area was exposed to respirable dust containing 3.7 percent crystalline silica (quartz) at an 8-hour TWA of 6.05 mg/m3, approximately 3.45 times the PEL of 1.75 mg/m3; this limit was established to prevent silicosis.  The employee was wearing a filtering face piece respirator and no respiratory protection program was written.         b)  Paint Booth, employees were required to use tight fitting respirators while spray painting paints and varnish exposing employees to inhalation hazards without a written respiratory protection program.
