Employee was not working within the basket as required:    Construction site: On or about January 9, 2015, and at times prior thereto, two employees were not standing firmly in the basket, one employee would climb the rails to access the roof to work and the second employee was working from the top and mid rails. This condition exposed the employees to a fall hazard greater than 6-feet.
