29 CFR 1910.178(l)(1)(i): The employer did not ensure that each powered industrial truck operator is competent to operate a powered industrial truck safely, as demonstrated by the successful completion of the training and evaluation specified in this paragraph (l):    a) Transervice Maintenance Area:  On or about January 9, 2015  The mechanics must operate powered industrial trucks after maintenance but have not been trained in accordance with the standard.

29 CFR 1910.334(a)(2)(ii): If there is a defect or evidence of damage that might expose an employee to injury, the defective or damaged item shall be removed from service, and no employee may use it until repairs and tests necessary to render the equipment safe have been made.      a) Transervice Maintenance Area:    On or about January 9, 2015    The Makita Saw was plugged in by extension cord.  The extension cord was damaged and missing the exterior cover.
