29 CFR 1910.119(j)(6)(i): In the construction of new plants and equipment, the employer did not assure that equipment as it was fabricated was suitable for the process application for which it was used: a) Lima Refining Company - Lima, Ohio: On or about January 10, 2015, the employer did not ensure that appropriate quality assurance checks and inspections had been documented for the fabrication of the Isocracker 1st Stage Reactor Effluent Air Cooler (REAC) (PR150278) prior to commissioning. Pre-installation inspection and testing for ferrite content of each weld and preheat and cooling rate temperatures were not documented in accordance with the design criteria specified for the fabrication of the REAC unit.
