Variety Contractors, Inc., Wal-Mart Hurricane WV: On or about 01/09/15, the scaffold was not fully planked when employees stocked masonry blocks on the top buck of the scaffold. ABATEMENT CERTIFICATION IS REQUIRED.

Variety Contractors Inc., Wal-Mart Hurricane WV: On or about 1/9/15, employees working on the top buck of the scaffold were not protected from falling 20' to the ground. ABATEMENT CERTIFICATION IS REQUIRED.
