29 CFR 1910.147(c)(6)(ii): The employer did not certify that periodic inspections of the energy control procedures had been performed:     Employees perform servicing and maintenance activities on equipment including, but not limited to the Hantsho Press, Trash Compactor, 8 Unit Color King which had multiple energy sources including pneumatic, natural gas, hydraulic, electric, gravity were exposed to hazards associated with the unexpected energizing, release of stored energy or inadvertent start-up of equipment.   Periodic inspections of energy control procedures for equipment were not documented.    29 CFR 1903.19(c)(1) requires certification that the abatement of the above violation is complete.

29 CFR 1910.178(p)(1): Powered industrial truck(s) found to be in need of repair, defective, or in any way unsafe, the truck shall be taken out of service until it has been restored to safe operating condition:  On the production floor by the Sheet-Fed Department, the Yale forklift had an inoperable light, exposing employees to a struck by hazard.  29 CFR 1903.19(c)(1) requires certification that the abatement of the above violation is complete.

29 CFR 1910.215(a)(4): Work rest(s) on grinding machinery were not adjusted closely to the wheel with a maximum opening of one eighth inch:  Work Bench Area - Employees operating the Dayton 6" Bench Grinder were exposed to tool jamming hazards in that the work rest on the left side of the operator was adjusted to 3/8 inch from the grinding wheel.  29 CFR 1903.19(c)(1) requires certification that the abatement of the above violation is complete.
