On or about 1/12/2015 in the maintenance shop over the parts room area: The area over the parts room in the maintenance shop was used by the employer as a storage area for material such as but not limited to; engine stand, boxes of papers, and other material was not load approved and marked for the floor rating by an approved official.

The employer failed to remove the powered industrial truck from service until the repairs could be made.
