29 CFR 1910.146(c)(2): The employer did not inform exposed employees, by posting danger signs or by any other equally effective means, of the existence and location of and the danger posed by the permit spaces:   Ridge Creek Industries, LLC, Covington, GA - In the Treating Plant, the chemical tanks were considered permit required confined spaces. The tanks were not identified with a sign indicating the danger, on or about 1/7/15.

29 CFR 1910.146(c)(7)(iii): The employer did not document the basis for determining that all hazards in a permit space had been eliminated, through a written certification that contained the date, the location of the space, and the signature of the person making the determination:  Ridge Creek Industries, LLC, Covington, GA - In the Treating Plant, the chemical tanks were considered permit required confined spaces. An employee entered the MP 200 tank after being drained but did not document the basis of determining that all hazards had been eliminated through a written certification, on or about 11/14.

29 CFR 1910.1200(e)(1): The employer did not develop, implement, and/or maintain at the workplace a written hazard communication program which describes how the criteria specified in 29 CFR 1910.1200(f), (g), and (h) will be met:  Ridge Creek Industries, LLC, Covington, GA - Employees were potentially exposed to hazard from wood treating chemicals such as but not limited to, MP 200 containing Tebuconazole, orange pigment containing 2-aminoethanol, Cleanwood 45 containing 2-Octyl-2H-isothiazol-3, wood dust, oil, grease and welding gasses. A written hazard communication program was not developed, on or about 1/7/15.
