29 CFR 1910.134(e)(4)(i): The medical questionnaire and examinations were not administered confidentially during the employee's normal working hours or at a time and place convenient to the employee:  a)  On 01/15/2015, a Spray Foam employee's medical questionnaire had not been administered confidentially because the employer had kept a copy of the employee's completed medical questionnaire along with the employee's medical clearance form from the physician or other licensed health care professional (PLHCP).
