29 CFR 1910.215(b)(9): The distance between the abrasive wheel periphery(s) and the adjustable tongue or the end of the safety guard peripheral member at the top exceeded one fourth inch: (a) Motor and Machine Shop, middle of lathe machining area - On or about January 8, 2015, employees using the Baldor Electric 3/4 horsepower, 3600 RPM pedestal grinder, Serial Number P0103, were exposed to injury due to the absence of tongue guards on both the left and right sides of the grinder. The absence of these guards left openings between the abrasive wheels and their respective housing peripheries of 2 inches on the user's left and 1 1/2 inches on the right.

29 CFR 1910.242(b): Compressed air used for cleaning purposes was not reduced to less than 30 p.s.i., nor was effective chip guarding and personal protective equipment used: (a) Machine Shop, Lathe - Machining Area - On or about January 8, 2015, employees using unidentified brand compressed air nozzles at the following work stations were exposed to injury from 65 to 85 psig air pressure due to non-compliant homemade nozzle extensions: (1) at Sidney 20 inch Engine Lathe, (2) at Lathe #3, and (b) Motor Repair Work Bench - On or about January 9, 2015, employees using unidentified brand compressed air nozzles at the following work stations were exposed to injury from 65 to 85 psig air pressure due to non-compliant homemade nozzle extensions: (3) "Safety Blow Gun" with Alemite grease fitting tip, (4) Unidentified silver nozzle with Alemite grease fitting tip

Morrison Keyway Cutter - On or about January 8-9, 2015, employees were exposed to tripping on the following items lying on the concrete floor: air hoses, welding leads, and a large diameter electric cord.
